Through the LEAD Collaborative, Meritus Health worked to improve health equity issues by creating a program aimed at addressing social isolation. The medical center operating in Western Maryland, implemented a care caller program that connects volunteers with patients who signaled to their providers they are feeling lonely through weekly calls to check on folks, how they are doing, and if they have any needs.
